Following an FOI request to every NHS trust in the UK, to which 120 trusts replied, it emerged that 365,608 specimens were mislabelled before they arrived at the pathology laboratories.
In addition, 11,712 samples were incorrectly labelled [...]

Another problem that we face with poor quality of wristbands and patient identification is the idea of lost samples or mismatched samples and a lot of staff time is spent on chasing up exceptions and again each time you have an exception you put a delay in the processes and every time you have a delay you may delay a patient discharge by up to twenty four hours or even longer if things have to be repeated perhaps the following day. Itrsquo;s also inconvenient for patients because patients want to be going home in daylight hours and if things are delayed and they have to go home at ten or twelve at night, transport costs rise and itrsquo;s very inconvenient and not good clinical practice.</itunes:summary>

<itunes:summary>In part 6 of our interview with Dr Richard Jones we ask about the financial implications of printed wristbands and labels.

One of the major reasons for delayed discharge from hospitals is claimed to be delayed results. Anything that can improve the speed of turnaround in results to the labs and back to the wards, if we can get patients in and out sooner of the hospital system the sooner that theyrsquo;re well enough to go home then we can save lots of bed days and hence we can save lots of money. The other big savings that can be made through the use of this sort of barcoding technology is simple efficiency in staff time because we waste so much staff time in re-entering data and re-keying data that we could just cut out all that redundant activity.</itunes:summary>

<itunes:summary>In part 4 of our interview with Dr Richard Jones we ask his views on the patients perspective on wristbands.

There is an issue with wristbands that sometimes patients feel theyrsquo;re a bit de-humanising, particularly with barcodes on. But patients are now becoming aware of the safety issues themselves and confidence in the system depends on the quality of the whole service. Their expectation in the supermarket is that they will see a barcode or their expectation in other walks of life is that they would have positive identification, when theyrsquo;re in a safety critical environment like a hospital then I think theyrsquo;re becoming accepting of the need to have positive ID. It also lends quality to the whole sense of the organisation if things are done professionally and properly and the idea of a wristband that canrsquo;t be read, thatrsquo;s been hand written and itrsquo;s loose or falling off, it just doesnrsquo;t lend you the confidence that the system knows what itrsquo;s doing. So I think that patients, although there are concerns about the de-humanisation, are actually much more accepting now of seeing things done properly and the safety case and the quality case is overwhelming.</itunes:summary>

<itunes:summary>In part 3 of our interview with Dr richard Jones, we ask about the importance of barcodes.

Using barcode technology which wersquo;re all familiar with from supermarkets can cut down the time involved with handling specimens or handling drug delivery. If yoursquo;ve got positive identification of the patient then therersquo;s no need to hand write tubes or hand write a lot of the paperwork that happens, thatrsquo;s at the patient end. At the laboratory end if the laboratory gets the right information usually the patients number then we can automate all our processes for handling the specimen reception and handling the admin side of the laboratory work. At the moment in laboratories one of the big bottle necks is in actually handling the data, the speed of the sample analysis is relatively fast but often wersquo;re waiting to do the paperwork to get the results back to the wards. In a hospital, that matters, if you have time critical return of results, for example the four hour wait in A#38;E, if yoursquo;re waiting on results because it takes an hour to do the booking in yoursquo;ve wasted an hours worth of time. Similarly in general practice if we can get information for a patient more readily then we can improve the turn around of time and make life more convenient for everyone involved. From a laboratory costing point of view it does mean that we can save our staff time and we have great problems in recruiting staff and we want to use those staff to do more interesting and important jobs than simply typing work into computers, if we can automate it we can use their time much more productively.</itunes:summary>
